Below you will find information regarding your last payroll check, COBRA, portability of your current Guardian benefit plans, and 401(k). As always, you can contact us, but we wanted to put the information at your fingertips as well.
Your final paycheck will be issued during the normal pay cycle that you are currently paid. Any accrued, but unpaid PTO hours will be paid on this final check. A copy of your final paycheck stub will be sent to your home address of record.
Our COBRA provider, VOYA, will mail you important information regarding your COBRA qualified benefits in the upcoming weeks. All previous H+H Associates will receive this paperwork due to automatic enrollment and eligibility for the Employee Assistance Program (EAP). Therefore, you will receive this paperwork even if you did not subscribe to medical, dental or vision insurance. If you subscribe to benefits at H+H, your benefits coverage will remain in effect through the last day of the month that your employment ended (excluding disability insurance). Disability coverage will remain in effect through the last day of your employment with H+H.
If you are currently enrolled in Voluntary Life Insurance or any of the other Voluntary Benefits (Accident, Critical Illness, or Hospital Indemnity) through Guardian and would like to keep the policy in effect, please click on the link to download the flyer: Guardian Coverage Continuation Options.
Benefits Plan, Inc, our 401(k) Administrator, will be sending you information regarding your 401(k) balance and the options you have regarding the handling of those funds. You will only receive this paperwork if you are currently enrolled in 401(k). This paperwork will be sent to you a few weeks after your final payroll check is processed.
